Aluminum hydroxide (Colenticon Gel) is among the common antacids available over the counter that rapidly relieves symptoms of heartburn.
Aluminum hydroxide, Colenticon gel syrup uses:
It is also helpful to relieve symptoms caused by:
- Peptic ulcer disease
- Hiatal hernia
- Gastritis, and
- Esophagitis.
Contraindicaions to Aluminum hydroxide, Colenticon gel
Patients Allergic to aluminum hydroxide should not use the drug.
Antacids Warnings and Precautions:
Aluminum hydroxide should be used with caution by patients with these conditions:
- Patients suffering from constipation or reduced bowel movements.
- Dehydration
- Diarrhea
- Appendicitis
- Occlusion of the gastric outlet
- Patients with upper gastrointestinal bleeding
- Dialysis encephalopathy may be caused by renal impairment
- Hypermagnesemia
Patients may also develop aluminum toxicities, such as osteomalacia or osteoporosis hypophosphatemia, which can lead to anorexia and muscle weakness, malaise, and even osteomalacia. [/bg_collapse]
Aluminum hydroxide, Colenticon gel Dose in Adults
Aluminum hydroxide, Colenticon Gel syrup dose for acidity:

Aluminum hydroxide Liquid (and magnesium hydroxide) 200 mg per 5 mL:
- 10-20 mL 4 times per day (maximum dose: 80 mL/day)

Aluminum hydroxide Suspension (and magnesium hydroxide) 500 mg per 5 mL:
- 10-20 mL 4 times per day, (maximum dose: 45 mL per day)

Tablet formulation (aluminum hydroxide 300 mg and magnesium hydroxide 150 mg):
- 1-2 tablets after meals or at bedtime, or as needed (maximum dose: 16 tablets/day)
Aluminum hydroxide, Colenticon gel Dose in Childrens
Note: Children should not use antacids for a longer duration

Liquid aluminum hydroxide 200 mg (and magnesium hydroxide 200 mg) per 5 mL:
- Children less than 12 years of age: 0.5-1 mL/kg/dose after meals and at bedtime (maximum dose: 15 mL)

Children greater than 12 years and dose in Adults:
- 10-20 mL 4 times per day (maximum daily dose: 80 mL per day)

Suspension of aluminum hydroxide 500 mg (and magnesium hydroxide 500 mg) per 5 mL:
- Children greater than 12 years of age and Adults: 10-20 mL 4 times per day, between meals and at bedtime (maximum dose in a day: 45 mL per day)

Tablet aluminum hydroxide 300 mg (and 150 mg magnesium hydroxide):

Children 6 to 11 years of age:
- 1-2 tablets 4 times per day

Children greater than 12 years and Adults:
- 1-2 tablets after meals and at bedtime, or as needed (the maximum daily dose should not exceed 16 tablets)

Aluminum hydroxide dose in kidney disease:
- Antacids especially aluminum hydroxide may accumulate in patients with renal diseases and cause nephrotoxicity.
- No dosage adjustment has been provided by the manufacturer, however, patients with a creatinine clearance of less than 25 ml/minute should take the drug with caution.
- Patients on dialysis may also develop dialysis encephalopathy with the chronic use of the antacid containing aluminum hydroxide.
Aluminum hydroxide dose in Liver disease:
- The manufacturer does not provide any adjustment in liver disease, however, salt restriction should be emphasized to prevent worsening of ascites
How to take Colenticon gel (Aluminum hydroxide):
Liquids and suspension:
- Before using the liquid or suspension formulation, shake it well and administer 1-2 hours after meals when stomach acidity is the highest.
Tablet formulation:
- Allow the tablets to dissolve in the mouth or chew it before swallowing. Take a glass of water after taking the antacid.
Pregnancy Risk factor: Not assigned
- Aluminum hydroxide containing antacids are generally considered safe during pregnancy and lactation when used occasionally for the treatment of dyspepsia.
- Although it has not been shown to cause teratogenic effects and does not enter breast milk, pregnant women and nursing mothers should avoid prolonged use of antacids.
Severe side effects of antacids (Colenticon Gel):
Delayed: Gastrointestinal obstruction
Moderate colenticon gel side effects:
- Early: Impaired cognition
- Delayed: Hemorrhoids, constipation, osteomalacia, encephalopathy, osteoporosis, hypophosphatemia, nephrolithiasis, and hypercalcemia
Mild Colenticon Gel Side effects:
- Early: Weakness, and malaise.
- Delayed: Anorexia
Monitoring Parameters:
- Phosphate levels, calcium and bone density in patients using long-term antacids.
Aluminum Hydroxide MOA:
- Hydrochloric acid reacts with aluminum hydroxide to create aluminum chloride and water. This neutralizes gastric acid secretions.
- The elevated PH prevents the production and action of pepsin which heals peptic ulcers.
- The lower esophageal sphincter is also closed by an increase in PH.
- This pressure is increased by the increased PH. It helps to manage reflux esophagitis.
- Taken with food, aluminum hydroxide blocks phosphate absorption. It can be used to treat hyperphosphatemia, hyperparathyroidism and calcinosis universalis.
