Indium In-111 Capromab Pendetide (Prostascint) - Uses, Dose

A prostate-specific membrane antigen-targeting IgG1 monoclonal antibody with a murine origin known as Prostascint is used in imaging procedures.

Indium In-111 capromab pendetide Uses:

  • Diagnostic imaging:

    • It is indicated for the diagnostic imaging in patients with:
      • Those with a recent diagnosis of biopsy-proven prostate cancer who have undergone conventional diagnostic testing and are at a high risk of developing pelvic lymph node metastases

      • Patients with postprostatectomy increased PSA levels and negative results from the conventional metastatic evaluation but strong clinical suspicions of metastatic disease

In diagnostic imaging, the dose of indium In-111 capromab pendetide is:

  • 0.5 mg intravenously radiolabeled with respect to 5 mCi of indium 111 chloride
  • The dose may be readministered if infiltration or the scan is technically inadequate.

Use in Children:

Not indicated.

  • Immunologic:

    • Antibody development
  • Cardiovascular:

    • Hypertension
    • Hypotension
  • Hepatic:

    • Increased serum bilirubin

 

Contraindications to Indium In-111 capromab pendetide:

Allergy reactions to Indium 112 capromab pendetide or other products from murine origin, Indium In111 chloride, or any component of this formulation

Warnings and precautions

  • Hypersensitivity

    • Patients can develop anaphylaxis or allergic reactions if they are receiving murine antibody.
    • To treat allergic or anaphylactic reactions, support medications such as adrenaline, hydrocortisone and antiallergic medicine must be readily available at the injection site.
  • Anti-human murine antibodies (HAMA).

    • Patients may develop anti-murine antibodies after receiving treatment that could affect the efficacy and effectiveness of murine antibody-based drugs. Further treatment may increase the risk of adverse reactions.
    • It is possible that murine-based products may not be used in the future to diagnose a recurrence. It may also affect some laboratory results.

Monitor for allergic reactions, syncope, and hypotension.

How to administer Indium In-111 capromab pendetide?

  • It is to be administered intravenously for about five minutes. The drug should not be mixed with any other medicines.
  • The night before the imaging session, a cathartic should be taken, and an hour before each of the 72–120 imaging sessions, a cleansing enema should be used.
  • Cathetrize and irrigate the bladder. Following the first infusion of the drug, an imaging session may be done after about 30 minutes.
  • The second imaging session may be done 72 to 120 hours after the infusion. The drug should be handled and disposed of appropriately since it is a radiopharmaceutical product.

Capromab pendetide is a monoclonal antibody from the murine IgG1 type directed at the glycoprotein PSMA (Prostate Specific Membrane Antigen) expressed in prostate epithelial cells.

  • Terminal: ~56 to 78 hours

  • Urine (10% over 72 hours)
