How can we differentiate between Bartter syndrome and Liddle syndrome?
Both conditions present with hypokalemia.
In bartter syndrome there is hypokalemia with normotension or hypotension and low sodium level while in liddles syndrome, patients have hypokalemia with hypertension and hypernatremia.
Calcium may be low in bartter and normal in liddle.
The pattern of inheritance in liddle is Autosomal dominant and for bartter syndrome autosomal recessive.
In bartter syndrome, aldosterone levels are high while aldosterone levels are low in liddle syndrome.
Pathogenesis:
Bartter: Multiple defects in Thick ascending loopf of Henle.
Liddle: Distal convuluted tubule and collecting duct defect, gain of function mutation ENAC.
