Amoxicillin is a beta-lactam antibiotic used to treat a variety of infections that include mostly Gram-Positive organisms and anaerobic bacterial organisms. Amoxicillin dose may vary in different infections. These factors include:
- The severity of infection
- The site of infection, and
- The bacterial organism causing the infection.
Amoxicillin is indicated for the treatment of the following infections:
- ENT infections like pharyngitis, tonsillitis, and otitis media in patients aged 12 years of age or more, caused by:
- Beta-lactamase-negative Streptococcus spp
- Streptococcus pneumoniae,
- Staphylococcus spp., or
- Haemophilus influenzae.
- Streptococcus pyogenes
- Genitourinary tract infections caused by:
- Beta-lactamase-negative Escherichia coli
- Proteus mirabilis
- Enterococcus faecalis.
- Helicobacter pylori eradication in combination therapy in patients with a duodenal ulcer
- Lower respiratory tract infections like pneumonia and bronchitis caused by:
- Beta-lactamase-negative Streptococcus spp.
- S. pneumoniae,
- Staphylococcus spp., or
- H. influenzae.
- Acute bacterial rhinosinusitis caused by:
- Betalactamase-negative Streptococcus spp.
- S. pneumoniae,
- Staphylococcus spp.
- H. influenzae.
- Skin and soft tissue infections caused by:
- Beta-lactamase-negative Streptococcus spp.
- Staphylococcus spp.
- E. coli.
- Actinomycosis, Anthrax, and prophylaxis against bacterial infection in high-risk patients
- Bronchiectasis, Prophylaxis of bacterial endocarditis, Lyme disease, Generalized Aggressive Periodontitis, and Prosthetic joint infection.
Amoxicillin Dose in Adults
The usual amoxicillin dose:
- Immediate-release: 500 mg to 1 g twice or thrice daily
- Extended-release: 775 mg once daily
Off-label use in the treatment of Actinomycosis:
Note: Severe infections should be treated with intravenous formulation followed by oral. Less severe infections may be treated with oral amoxicillin.
- Oral amoxicillin: 500 mg thrice or four times a day. Higher doses of 4 to 6 g daily have been used in case reports. Treatment duration depends on the severity and extent of the disease and may vary from 2 - 12 months.
Penicillin-susceptible strains of Anthrax:
Inhalational exposure and post-exposure prophylaxis: Oral amoxicillin 1 gm thrice daily for 60 days.
Cutaneous anthrax:1 gm orally thrice daily for 60 days (following infections acquired via biological weapons), and 10 days following naturally acquired infection.
Note: Public health specialists should be consulted. Exposed individuals should be administered anthrax vaccine.
Amoxicillin dose in the prevention of bacterial infections in high-risk patients with asplenia:
- 500 mg orally twice daily.
Amoxicillin dose in Bronchiectasis:
- Treatment of pulmonary exacerbations: 500 mg to 1000 mg thrice daily for 14 days.
- Prevention of pulmonary exacerbations (in patients with more than 3 exacerbations per year who are not colonized with pseudomonas aeruginosa: 500 mg orally twice daily.
Amoxicillin dose for the prophylaxis of endocarditis (prior to dental or invasive respiratory tract procedures):
- 2 gms 30 to 60 minutes before the procedure.
Amoxicillin dose in the eradication of Helicobacter pylori infection:

Clarithromycin triple regimen:
- Amoxicillin 1 g twice daily for 14 days in combination with clarithromycin 500 mg twice daily, plus a standard-dose or double-dose proton pump inhibitor.

Concomitant regimen:
- Amoxicillin 1 g twice daily for 14 days in combination with clarithromycin 500 mg twice daily, either tinidazole or metronidazole 500 mg twice daily, plus a standard-dose proton pump inhibitor twice daily.

Sequential regimen:
- Amoxicillin 1 gm twice daily plus a standard-dose proton pump inhibitor twice daily for 7 days; followed by clarithromycin 500 mg twice daily and either metronidazole or tinidazole 500 mg twice daily, plus a standard-dose proton pump inhibitor twice daily for 5 - 7 days.

Hybrid regimen:
- Amoxicillin 1 g twice daily plus a proton pump inhibitor twice daily for 14 days. Add clarithromycin 500 mg twice daily and either metronidazole or tinidazole 500 mg twice daily after 7 days of therapy for 7 days.

Levofloxacin triple regimen (salvage regimen):
- Amoxicillin 1 g twice daily in combination with levofloxacin 500 mg once daily and a standard-dose proton pump inhibitor twice daily for 10 to 14 days.

The Salvage regimen (High-dose dual therapy):
- Amoxicillin 750 mg 4 times a day or 1 gm thrice daily in combination with a proton pump inhibitor 3 to 4 times daily for 14 days
- Amoxicillin 750 mg 4 times a day or 1 gm thrice daily in combination with a proton pump inhibitor 3 to 4 times daily for 14 days
Off label use in Lyme disease:

Patients with early and localized disease (eg, erythema migrans):
- 500 mg thrice daily for 14 to 21 days

Patients with disseminated disease (carditis and neurologic involvement without meningitis):
- 500 mg thrice daily for 21 days.

Late disease with arthritis & without neurologic involvement:
- 500 mg 3 times daily for 28 days
- 500 mg 3 times daily for 28 days
Amoxicillin dose in Otitis media:
- 500 mg thrice daily or 875 mg every twice daily for 7 to 10 days. Amoxicillin/clavulanic acid may be preferred over amoxicillin)
Amoxicillin dose in generalized aggressive periodontitis:
- 250 to 500 mg thrice daily in combination with metronidazole for 14 days (Wilder 2018)
Amoxicillin dose in community-acquired pneumonia (CAP):
- 1000 mg thrice daily in combination with other appropriate medicines like macrolides for a minimum of five days. Therapy may be discontinued when the patient is afebrile for at least two days (IDSA/ATS [Mandell 2007).
Prosthetic joint infection:
For chronic suppression of bacterial growth in patients (who are not candidates for removal of the prosthesis) with infected prosthetic joints caused by beta-hemolytic streptococci, penicillin-susceptible Enterococcus spp., or Cutibacterium spp.
- 500 mg thrice daily (IDSA [Osmon 2013) (duration depends on patient-specific factors)
Amoxicillin dose in Acute bacterial Rhinosinusitis:
- 500 mg thrice daily or 875 mg twice daily for 7 days in patients aged less than 65 years and without any risk factors like diabetes and an immunocompromised state.
Amoxicillin dose in Skin and soft tissue infection:
- Mild Erysipelas:
- 500 mg thrice daily or
- 875 mg twice daily for 5 to 14 days (longer duration may be used in patients with more severe infection or in immunosuppressed individuals).
Amoxicillin dose for Group A Streptococcal pharyngitis:
- 500 mg twice daily or 1 g once daily for 10 days (AHA [Gerber 2009]
- Extended-release: 775 mg once daily for 10 days

Amoxicillin dose for prophylaxis of bacterial endocarditis:
(Prophylaxis of endocarditis should be limited to high-risk patients and those with potential adverse outcomes including:
- Prosthetic heart valves
- Patients with previous IE
- Patients with unrepaired cyanotic congenital heart disease
- Those with repaired congenital heart disease during the first six months after the procedure or with prosthetic material
- Repaired congenital heart disease (with residual defects)
- Heart transplant recipients with cardiac valvulopathy.
- Infants, Children, and Adolescents:
- 50 mg/kg 30 to 60 minutes prior to the procedure (maximum dose is 2,000 mg/dose)

Helicobacter pylori eradication therapy:
- Children and Adolescents:
- 50 mg/kg/day in twice daily for 14 days (in combination with a proton pump inhibitor or bismuth subsalicylate and either clarithromycin or metronidazole)
- 50 mg/kg/day in twice daily for 14 days (in combination with a proton pump inhibitor or bismuth subsalicylate and either clarithromycin or metronidazole)
Amoxicillin dose in Lyme disease:
- Infants, Children, and Adolescents:
- 50 mg/kg/day in three divided doses to a maximum dose of 500 mg/dose
- 50 mg/kg/day in three divided doses to a maximum dose of 500 mg/dose

Mild infection caused by Group A Streptococcus:
- 50 to 75 mg/kg/day in two divided doses to a maximum of 4000 mg per day

Mild infection caused by Haemophilus influenzae:
- 75 to 100 mg/kg/day in three divided doses to a maximum of 4 gms daily

Mild infection caused by Streptococcus pneumonia (penicillin MIC ≤2 mcg/mL):
- 90 mg/kg/day in two divided doses every 12 hours or
- 45 mg/kg/day in three divided doses every 8 hours
- The maximum daily dose is 4,000 mg/day
Prophylaxis of pneumococcal infection in patients with functional or anatomic asplenia:

- 20 mg/kg/day in two divided doses every 12 hours to a maximum dose of 250 mg/dose.

Amoxicillin dose in acute uncomplicated bacterial rhinosinusitis:
- Children older than 2 years and Adolescents:
- 45 mg/kg/day in two divided doses
- 80 to 90 mg/kg/day in two divided doses to a maximum dose of 2000 mg/dose for moderate to severe infections
Amoxicillin dose for the treatment of Tonsillopharyngitis caused by Group A streptococcal infection and prevention of rheumatic fever:
- Children and Adolescents 3 to 18 years:
- 50 mg/kg once daily or 25 mg/kg twice daily for 10 days to a maximum dose of 1 gm daily.
- 775 mg extended-release formulation once daily may be given to children 12 years of age or more.
Prophylaxis of UTI in patients with hydronephrosis and vesicoureteral reflux:
- Infants 2 months of age or less:
- 10 to 15 mg/kg once daily in the evening.
Pregnancy Risk Factor B
- Amoxicillin crosses the placenta, however, teratogenic effects have not been observed in animal and human studies.
- Rare case reports of cleft lip and cleft palate have been observed.
- Amoxicillin may be used to treat anthrax in pregnancy, management of PROM, prophylaxis of endocarditis, and as an alternative treatment of chlamydial infection.
Amoxicillin use in Breastfeeding
- Although amoxicillin is excreted in breast milk, it is generally considered acceptable.
- Allergic reactions, thrush, and diarrhea due to altered bowel flora in infants must be monitored.
- Amoxicillin may also be used to treat mastitis and anthrax in breastfeeding women.
Amoxicillin Dose in Renal Disease:

Immediate-release oral formulation:
- Avoid the 875 mg dose in patients with an eGFR of less than 30 ml/min.

Creatinine clearance of 30 mL/minute or more:
- Dose adjustment is not necessary.

Creatinine clearance of 10 to 30 mL/minute:
- 250 to 500 mg twice daily

Creatinine clearance of less than 10 mL/minute:
- 250 to 500 mg once daily

Hemodialysis:
- Amoxicillin is moderately dialyzable. The dose should be administered after dialysis on the dialysis day: 250 to 500 mg once daily.

- No dosage adjustment is necessary according to the manufacturer's labeling.

The manufacturer has not advised any dose adjustment in patients with liver disease.

- Acute generalized exanthematous pustulosis, erythematous maculopapular rash, exfoliative dermatitis, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, and urticaria.

Gastrointestinal:
- C.difficile associated diarrhea/ colitis, hemorrhagic colitis, melanoglossia, and mucocutaneous candidiasis

Genitourinary:
- Crystalluria

Hematologic & oncologic:
- Agranulocytosis, anemia, eosinophilia, hemolytic anemia, immune thrombocytopenia, thrombocytopenia, and leukopenia.

Hepatobiliary:
- Cholestatic jaundice, hepatitis (acute cytolytic), increased serum ALT and AST.

Hypersensitivity:
- Anaphylaxis

- Serum sickness-like reaction
Contraindications to amoxicillin:
- Severe allergic reactions (like anaphylaxis, Steven-Johnson syndrome or TEN) to amoxicillin or any component of the formulation.
- Other beta-lactams can cause serious hypersensitivity reactions
- Cases of infectious mononucleosis suspected or confirmed.
Warnings and Precautions
-
Superinfection:
- Amoxicillin prolonged use can lead to the growth of bacteria and fungi.
- Of importance are C.difficile-associated diarrhea and pseudomembranous colitis following prolonged (more than 2 months) use of amoxicillin.
-
Infectious mononucleosis
- Amoxicillin may cause a rash in patients with IM.
- Patients with CMV infection or Acute Lymphoblastic Leukemia may experience rash after amoxicillin is taken.
-
Renal impairment
- Patients with impaired renal function (eGFR less than 30ml/min) should not use it.
- Patients with ESRD or those on hemodialysis should avoid extended-release formulations.

Probenecid | Increases the serum level of Penicillins. Management: Penicillins should not be combined with probenecid. However, this combination can be beneficial in certain cases and under careful supervision. If probenecid is given or if the dose is increased, monitor for potential toxic effects. |
Sodium Picosulfate | Antibiotics can reduce the therapeutic effects of Sodium Picosulfate. Patients who are currently using or have just finished using antibiotics should consider using an alternative product to cleanse the bowels before undergoing a colonoscopy. |

Typhoid Vaccine | The therapeutic effects of Typhoid vaccine may be diminished by antibiotics. The only affected strain is the live attenuated Ty21a. This vaccine should not be used until at least three days after the cessation or discontinuance of antibacterial agents. |

Monitoring parameters:
- With prolonged Amoxicillin therapy, renal, hepatic, and hematologic functions should be monitored periodically.
- Clinical features of anaphylaxis should be observed after the administration of the first amoxicillin dose. The patient should be evaluated periodically for the response to therapy.
Amoxicillin dose administration in adults:
- The drug should be administered around-the-clock to avoid variations in the plasma levels.
- The extended-release formulation should be administered within one hour of finishing a meal. The tablets and capsules should not be crushed or chewed.
- The oral suspension should be shaken well before use. It may be mixed with formula, milk, fruit juice, water, ginger ale, or cold drinks.
Amoxicillin dose administration in children:
- Amoxicillin may be administered on an empty or full stomach. It may be mixed with formula, milk, cold drink, or juice.
- The suspension should be shaken well before use and the dose should be administered after mixing.
- The extended-release formulation should be administered within one hour of a meal.
- The tablets and capsules should be swallowed whole. Crushing or chewing the tablets should be avoided.
Mechanism of action of Amoxicillin:
- It inhibits the cell wall synthesis by binding to penicillin-binding protein (PBPs), which in turn inhibits peptidoglycan production in bacterial cell walls.
- This causes the death of the bacteria.
AbsorptionThe oral immediate-release formulation is faster than the extended-release. The absorption rate is affected by food.
It isdistributedIt can be absorbed easily and quickly into the liver, lungs and prostate, muscle, middle-ear effusions and maxillary sinus secretions, bone and gallbladder, as well as into synovial and ascitic fluids.
