Cefotaxime (Claforan) is a third-generation cephalosporin like Ceftriaxone that is used to treat the following bacterial infections:
- Treatment of Bacteremia or Septicemia caused by:
- Escherichia coli,
- Klebsiella species,
- Serratia marcescens,
- Staphylococcus aureus and
- Streptococcus species including Streptococcus pneumoniae.
- Treatment of Bone and joint infections caused by:
- Penicillinase and non-penicillinase producing strains of S. aureus,
- Streptococcus species including Streptococcus pyogenes, and
- Proteus mirabilis.
- Treatment of CNS infections like meningitis and ventriculitis caused by:
- Neisseria meningitidis,
- Haemophilus influenzae,
- S. pneumoniae,
- Klebsiella pneumoniae,
- E. coli.
- Treatment of Genitourinary infections (including Urinary tract infections caused by:
- Staphylococcus epidermidis,

Cefotaxime Dose in Adults
Off-label use of Cefotaxime in treatment of severe Acute bacterial rhinosinusitis that necessitates hospitalization:
- 2 g intravenous every 4 - 6 hours for 5 - 7 days.
Off label use of Cefotaxime in the treatment of animal Bite wound:
- 1 to 2 g intravenous three or four times a day in combination with clindamycin or metronidazole for anaerobic coverage
Use of Cefotaxime in the empiric treatment of Brain abscess:
- 2 gms intravenous four or six times a day in combination with other antimicrobials (like metronidazole)
Dose in patients undergoing a Cesarean section:
- As soon as the umbilical chord is clamped, administer 1 g intravenously. Then, administer 1 gm intravenously or intramuscularly at 6 and 12 hours later.
Use of a different medication off-label to treat disseminated Gonorrhea infections, such as arthritis and the arthritis-dermatitis syndrome:
- 1 gm intravenous thrice daily in combination with azithromycin for 24 to 48 hours. Oral antibiotics may replace IV ceftriaxone after improvement and according to susceptibility testing.
- Treatment should be given for at least 7 days.
Use in the treatment of mild to moderate Intra abdominal community-acquired infection like a perforated appendix, diverticulitis, and intraabdominal abscess in the low-risk patient.
- 2 gms intravenous thrice daily in combination with metronidazole for 4 to 7 days.
- Oral therapy may replace IV therapy when the patient improves and is able to tolerate orally.
- Some patients may require a longer duration of therapy.
Off-label use as an alternative agent for Lyme disease:
- Cardiac manifestations:
- 2 gms intravenous thrice daily for 14 - 21 days
- Neurologic manifestations like meningitis and radiculopathy:
- 2 gms intravenous thrice daily for 10 - 28 days.
Use in treatment of bacterial Meningitis:
- 2 gms intravenous four to six times a day in combination with other appropriate antimicrobials when used as empiric therapy.
Use of Cefotaxime in the treatment of Community-acquired Pneumonia (CAP):

- 1-2 gms intravenous thrice daily for 5 or more days.
- In critical patients use in combination with azithromycin or a respiratory fluoroquinolone.
- In non-critically, ill patients use in combination with a macrolide or doxycycline
Cefotaxime dose in the treatment of Sepsis:

- 2 gms intravenous three to four times a day
Use in the treatment of Septic arthritis:
- Neisseria gonorrhoeae:
- 1 gm intravenous three times a day for 7 to 14 days in combination with a single dose of azithromycin
- Susceptible gram-negative bacilli:
- 2 gms intravenous three times a day for 3 to 4 weeks in the absence of osteomyelitis.
Off label use in the treatment of Skin and soft tissue necrotizing infections:
- Polymicrobial infection:
- For the initial treatment of polymicrobial infections, provide 2 gms intravenously four times per day in conjunction with clindamycin or metronidazole.
- It should be continued until the patient improves, is afebrile for 48 to 72hours and further debridement is not necessary.
- Necrotizing infection due to Vibrio vulnificus:
- 2 gms intravenous thrice daily in combination with doxycycline.
- It should be continued until the patient improves, is afebrile for 48 to 72hours and further debridement is not necessary.
Off label use in the treatment of Spontaneous bacterial peritonitis:

- 2 gms intravenous thrice daily.
Off label use in the treatment of Surgical prophylaxis:
-
- 1 gm intravenous one hour prior to the surgical incision.
- The dose may be repeated in lengthy procedures (more than 3 hours or when excessive blood is lost)
- Obese patients may be administered a dose of 2 gms one hour prior to the procedure.
Cefotaxime Dose in Children
General dosing of Cefotaxime in susceptible infection:
- Infants, Children, and Adolescents:
- 150 to 180 mg/kg/day intramuscular or intravenous in three divided doses to a maximum dose of 8 gms/day
- Patients with meningitis may require a higher dose.
Use in treatment of severe Acute bacterial rhinosinusitis that necessitates hospitalization:
- Children and Adolescents:
- For a total dose of 2,000 mg, provide 100 to 200 mg/kg/day intravenously in four evenly spaced doses for 10 to 14 days.
Cefotaxime dose in the treatment of Endocarditis:
- Children and Adolescents:
- 200 mg/kg/day intravenous in four divided doses to a maximum dose of 12 g/day 4 to 6 weeks.
- It may be used for a longer duration in some patients and may be combined with gentamicin for some organisms.
Use of cefotaxime in HIV positive or HIV exposed patients as empiric treatment for Enteric bacterial infections:
- Adolescents:
- 1,000 mg intravenous thrice daily
Use as an alternative to ceftriaxone in the treatment of disseminated Gonorrheal infection including arthritis and arthritis-dermatitis syndrome:
- Adolescents:
- 1,000 mg intravenous thrice daily in combination with azithromycin for at least 7 days.
Cefotaxim dose in the treatment of complicated Intra-abdominal infection:
- Infants, Children, and Adolescents:
- 150 to 200 mg/kg/day intravenous in three or four divided doses to a maximum dose of 2 gms in combination with metronidazole.
Use in treatment of Lyme disease (with cardiac manifestations, CNS manifestations or recurrent arthritis):
- Infants, Children, and Adolescents:
- 150 to 200 mg/kg/day intravenous in three or four divided doses for 14 - 28 days to a maximum daily dose of 6 g/day.
Cefotaxime dose in the treatment of Meningitis:
- Infants, Children, and Adolescents:
- 225 to 300 mg/kg/day intravenous three or four times a day to a maximum dose of 2 gms per dose.
- Use vancomycin in conjunction with for empiric treatment.
Some professionals advise 300 mg/kg/day administered every four to six hours, with a daily dose cap of 12 g.
Use in treatment of Peritonitis in patients on peritoneal dialysis:
- Infants, Children, and Adolescents:
- Patients on intermittent peritoneal dialysis: 30 mg/kg/dose intraperitoneal once a day.
- Patients on continuous peritoneal dialysis:
- An intraperitoneal loading dose of 500 mg per liter of dialysate
- maintenance dose of 250 mg per liter
Use of Cefotaxime in the treatment of Pneumonia:
- Bacterial pneumonia in HIV-exposed or positive patients:
- Infants, Children, and Adolescents:
- 150 to 200 mg/kg/day intravenous in three or four divided doses to a maximum of 2 gms//dose
- Infants, Children, and Adolescents:
- Community-acquired pneumonia (CAP):
- Infants older than 3 months, Children, and Adolescents:
- 50 mg/kg/dose intravenous thrice daily to a maximum dose of 2 gms.
- The addition of vancomycin or clindamycin may be added as empiric therapy if community-acquired MRSA is suspected.
- Macrolide antibiotics that cover the atypical organisms may be added in children older than 5 years of age
- Infants older than 3 months, Children, and Adolescents:
Use in the treatment of Salmonellosis in HIV-exposed or HIV- positive patients:
- Adolescents:
Use of Cefotaxime in the treatment of necrotizing Skin and soft tissue infections:
- Infants, Children, and Adolescents:
- 50 mg/kg/dose four times a day in combination with metronidazole or clindamycin to a maximum dose of 2 gms/dose.
- It should be continued until the wound is healthy, the patient is afebrile for more than 48 to 72 hours and further debridement is not necessary.
Use of Cefotaxime in the treatment of Surgical prophylaxis:
- Children and Adolescents:
- 50 mg/kg intravenous one hour prior to the procedure. The dose may be repeated in the lengthy procedure (more than 3 hours)
- The maximum recommended dose is 1 gm, however, obese patients may be given 2 gms.
Use of Cefotaxime in the treatment of Urinary tract infection:
- Infants and Children 2 - 24 months of age:
- 150 mg/kg/day IM or IV in three or four divided doses
Pregnancy Risk Factor B
- Cefotaxime(Claforan).Crosses the placental boundary, but there have been no adverse drug reactions when taken during pregnancy.
- It can be used by women who are having a cesarean section
Cefotaxime use during breastfeeding:
- Cefotaxime can be found in breastmilk but is compatible with breastfeeding. It can alter the baby's bowel flora, and cause GI problems in the infant.
Cefotaxime Dose in Renal Disease:
Renal dose adjustment according to the manufacturer:
- CrCl of less than 20 mL/minute/1.73 m²: Reduce the dose by 50%. It is moderately dialyzable (20 - 50%)
Alternate recommendations for dose adjustment in patients with kidney disease:
- GFR of more than 50 mL/minute: Administer the dose every 6 hours
- GFR of 10 - 50 mL/minute: Administer the dose twice daily
- GFR of less than 10 mL/minute: Administer the dose once daily or reduce the dose by 50% at the usual recommended intervals
- Patients on intermittent hemodialysis: Administer 1 - 2 gms once daily after hemodialysis
- Patients on peritoneal dialysis: Administer 1 gm once daily
- Patient with CVVH: 1 to 2 g twice or thrice daily
- Patient with CVVHD: 1 to 2 g thrice daily
- Patient with CVVHDF: 1 to 2 g three or four times a day.
Cefotaxime Dose in Liver Disease:
The manufacturer has not recommended any dose adjustment in patients with liver disease.

- Dermatologic:
- Pruritus
- Skin rash
- Gastrointestinal:
- Colitis
- Diarrhea
- Nausea
- Vomiting
- Hematologic & oncologic:
- Eosinophilia
- Local:
- Induration at injection site
- Inflammation at injection site
- Pain at injection site
- Tenderness at injection site
- Miscellaneous:
- Fever
Contraindications to Cefotaxime include:
- Allergy reactions to cefotaxime or other cephalosporins or any component of this formulation
Warnings and Precautions
- Arrhythmia:
- Rapid injections (less than one-minute) via a centralline can cause potentially life-threatening arrhythmia.
- Granulocytopenia:
- Cytopenias may develop if prolonged therapy is continued for more than 10 days. This could include granulocytopenia or life-threatening agranulocytosis.
- Penicillin allergy
- Patients who experience IgE-mediated responses like anaphylaxis, urticaria, or angioedema should use cefotaxime with caution.
- Superinfection
- Cefotaxime treatment for longer periods (over 2 months) can lead to superinfections, including fungal or bacterial superinfections. C. difficile-associated diarrhea, (CDAD), and pseudomembranous collitis can occur.
- Local inflammation
- Local site inflammation can occur at the injection/infusion site.
- Colitis
- Patients who have had colitis in the past should be cautious when taking this drug.
- Renal impairment
- Patients with kidney disease must be cautious when using the drug. The dose should also be adjusted.

Probenecid | There may be an increase in serum Cefotaxime concentrations. Management: When using a probenecid at the same time as cefotaxime, do not exceed 6 g/day. Patients receiving this combination should be carefully watched for any adverse effects of cefotaxime. |
Sodium Picosulfate | Antibiotics can reduce the therapeutic effects of Sodium Picosulfate. Patients who are currently using or have just finished using antibiotics should consider using an alternative product to cleanse the bowel prior to undergoing a colonoscopy. |
Typhoid Vaccine | The therapeutic effects of Typhoid vaccine may be diminished by antibiotics. The only affected strain is the live attenuated Ty21a. Patients being treated with systemic antibiotics should avoid vaccination with live attenuated Typhoid vaccine (Ty21a). This vaccine should not be used until at least three days after the cessation or discontinuation of antibacterial agent treatment. |

Monitoring Parameters:
- Monitor the patient for features of anaphylaxis, especially after the first dose.
- Monitor CBC with differential and renal functions especailly in patients on treatment for more than 10 days.
How to Administer Cefotaxime?
Intramuscular administration:
- It should be injected deep into a large muscle. Larger doses may be divided and injected separately in different sites.
Intravenous administration:
- It may be administered as an intravenous bolus over 3 - 5 minutes or as an infusion over 15 to 30 minutes.
Mechanism of action of Cefotaxime:
- Cefotaxime blocks the formation of bacterial proteins, peptidoglycans and penicillin-binding proteins.
- This eventually leads to cell lysis through the use of autolytic enzymes.
- Both gramme positive and gramme negative bacteria that produce beta-lactamases, such as penicillinases and cephalosporinases, are susceptible to cefotaxime.
- Bacteria may be naturally resistant to cefotaxime.
- Additionally, the majority of isolates that produce carbapenemase and ESBL-producing bacteria are cefotaxime resistant.
