Cialis (tadalafil) is a phosphodiesterase inhibitor. It relaxes the smooth muscles of the urogenital tract, respiratory vasculature. Cialis dosage for BPH and erectile dysfunction is discussed here. Cialis (tadalafil), like sildenafil and vardenafil, is used primarily for the treatment of erectile dysfunction.
Cialis dosage for BPH:
Patients with benign prostatic hyperplasia receive a daily dosage of 5 mg of Cialis. Alpha-blockers like prazosin and tamsulosin shouldn't be used concurrently with them. In addition, it must not be used with alprostadil (Caverject). Strong CYP3A inhibitors such clarithromycin, erythromycin, diltiazem, ketoconazole, itraconazole, verapamil, ritonavir, grapefruit, and goldenseal may be used by patients who are taking Cialis at a dose that does not exceed 2.5 mg per day.
Cialis dosage for BPH in kidney disease:
Cialis should not be taken by hemodialysis patients or anyone with GFRs less than 30 ml/min.
Patients who have a creatinine clearance of 30 to 50 ml/min or less can take the medication at half the usual dose, up to a daily maximum of 5 mg.
Cialis dosage for BPH in liver disease:
When a patient has mild to moderate liver illness, it should be taken with caution and avoided in cases of severe hepatic impairment.
Patients on vasodilators and those with ischemic heart disease should also avoid their use. It should not be used with nitrates. This includes all forms of nitrates including intermittent dosing, long-acting, and short-acting nitrates. A minimum of 48 hours should pass following the last dose of Cialis if the use of nitrates cannot be avoided (tadalafil). It should not be used for more than 26 weeks when used concomitantly with finasteride.
Cialis dosage in the treatment of Erectile dysfunction:
Tadalafil (Cialis) may be used on a once-daily basis or intermittently. Improvement in erectile dysfunction may last up to 36 hours. Some experts advise the drug on a once-daily dose and add a short-acting drug like sildenafil on weekends for better results. However, a combination of two similar drugs may cause harm and should not be tried without prior consulting an expert.

Cialis intermittent dosing for erectile dysfunction:
- The dose may be increased or reduced based on the individual's tolerability.
- The usual Cialis dose is 5mg to 20 mg
- 10 mg should be taken at least 30 minutes before anticipated sexual activity as one single dose. More than one dose on a single day should be avoided.

Daily Cialis dose:
- Without regard to when sexual activity takes place, 5 mg is administered once a day at the same time.
- The typical daily dosage is 2.5 to 5 milligrammes.
When administered concurrently, patients should be stable on a fixed dosage of alpha-blockers such as prazosin and tamsulosin. Furthermore, Cialis may be initiated at the lowest recommended dose. The maximum daily dosage should not exceed 10 mg per 72 hours when taken intermittently with powerful CYP3A4 inhibitors. When used on a daily basis, the maximum dose is 2.5 mg once a day.
Cialis intermittent dosage in kidney disease:
The dosage of Cialis in erectile dysfunction does not need adjustment in patients with kidney disease and a CrCl ≥51 mL/minute.
An initial dosage of 5 mg once day, which may be raised to a maximum dose of 10 mg every 48 hours, is used for those with a CrCl of 30 to 50 ml/min.
The maximum dosage should not exceed 5 mg every 72 hours for people on hemodialysis and those whose CrCl is less than 30 mL/minute.
Cialis dosage in kidney disease when used on a daily basis:
It is not advised to change the dosage for patients whose CrCl is greater than 30 ml/min.
Patients on hemodialysis and those whose CrCl is less than 30 ml/min should refrain from using it every day.
Cialis dosage in liver disease (intermittent or daily dose):
When treating individuals with mild to severe liver disease, it should be used cautiously (Child-Pugh class A or B). The daily intake shouldn't be more than 10 mg.
It should be avoided in cases of severe liver disease (Child-Pugh Class C).
The following drugs may increase the serum concentrations of Cialis (tadalafil). Monitoring with concomitant administration of the following drugs is recommended:
- Alcohol (may enhance the blood pressure lowering effects of Cialis)
- Aprepitant
- Clofazimine
- Cobicistat
- Conivaptan (Avoid concomitant use)
- Dapoxetine (It may enhance its hypotensive effect. Avoid concomitant use)
- Duvelisib
- Erdafitinib
- Fluconazole
- Fosaprepitant
- Fosnetupitant
- Idelalisib (Avoid concomitant use)
- Itraconazole
- ketoconazole
- Larotrectinib
- Lorcaserin (May enhance the adverse effects of Cialis especially priapism.
- Mifepristone
- Molsidomine
- Netupitant
- Nitroprusside (Avoid concomitant use because of the risk of hypotension)
- Palbociclib

- Ritonavir

The following drugs may decrease the serum concentrations of Cialis (tadalafil):
- Bosentan
- Etravirine
- Dexamethasone
- Carbamazepine
