Rocephin ( Ceftriaxone) is a third generation cephalosporin antibiotic that inhibits bacterial cell wall synthesis. It is indicated for the treatment of the following disorders:

Infection of the middle ear - Acute bacterial otitis media that is caused by:
- Streptococcus pneumoniae
- Haemophilus influenzae including strains producing beta-lactamase or
- Moraxella catarrhalis including strains producing beta-lactamase

Ceftriaxone Dose in Adults

- 1 to 2 g intravenous once or twice daily

- Patients on anticoagulants should avoid intramuscular injections.
Off label use in the treatment of native valve or prosthetic valve Endocarditis caused by
- Penicillin-susceptible and gentamicin-susceptible or penicillin-susceptible and aminoglycoside resistant enterococcus:
- 2 g intravenous twice daily for 6 weeks with concomitant ampicillin
- HACEK organisms:
- 2 g intravenous or intramuscular once a day for 4 weeks (native valve) or 6 weeks (prosthetic valve)
- Viridans group Streptococcus (VGS) and S. bovis:
- Native valve endocarditis: Highly penicillin-susceptible organsisms with of MIC ≤0.12 mcg/mL:
- 2 gms once a day for 4 weeks or
- for 2 weeks with concomitant gentamicin
- Prosthetic valve: Highly penicillin-susceptible organisms with a MIC of ≤0.12 mcg/mL:
- 2 gms once a day for 6 weeks with or without concomitant gentamicin for the first 2 weeks.
- Prosthetic valve: penicillin-resistant organisms with a MIC of >0.12 mcg/mL:
- 2 gms once a day with concomitant gentamicin for 6 weeks
- Native valve endocarditis: Highly penicillin-susceptible organsisms with of MIC ≤0.12 mcg/mL:
Off label use of ceftriaxone in the treatment of acute Epididymitis:
- Caused by chlamydia and gonorrhea transmitted via sexual route:
- 250 mg intramuscular as a single dose plus doxycycline
- Caused by sexually transmitted gonorrhea, chlamydia and enteric organisms in men who practice insertive anal sex:
- 250 mg intramuscular as a single dose plus oral levofloxacin or oral ofloxacin
Rocephin dose in Gonococcal infections:
- Off-label use in the treatment of gonococcal Conjunctivitis:
- 1 gm intramuscular as a single dose plus oral azithromycin
- Off label use in the treatment of gonococcal arthritis and Disseminated gonococcal infection:
- 1 gm intravenous or intramuscular once daily plus single dose oral azithromycin for 24 to 48 hours
- switch to an oral agent guided by antimicrobial susceptibility after clinical improvement for a total of at least 7 days of therapy.
- Off-label use in the treatment of gonococcal Endocarditis:
- 1 to 2 g intravenous once or twice daily plus oral azithromycin;
- Continue ceftriaxone for at least 28 days
- Off-label use in the treatment of gonococcal Meningitis:
- 1 to 2 g intravenous once or twice daily plus oral azithromycin;
- Continue ceftriaxone for 10 to 14 days.
- Uncomplicated gonococcal Cervicitis, proctitis, urethritis, and pharyngitis (off-label):
- Dual-therapy regimen (preferred regimen):
- 250 mg intramuscular as a single dose plus oral azithromycin
- Dual-therapy regimen (preferred regimen):
- For treatment failure, dual therapy is recommended as a combination of either ceftriaxone, gemifloxacin, or gentamicin combined with oral azithromycin.
- Treatment failure after the single-therapy regimen or other non-preferred treatment regimens:
- 250 mg intramuscular in a single dose plus azithromycin 1 gm orally as a single dose
- Treatment failure following initial therapy or an alternative dual-therapy regimen consisting of cefixime and azithromycin:
- 250 mg intramuscular in a single dose plus azithromycin 2 gms orally as a single dose.
- Treatment failure following the initial therapy or the preferred dual-therapy regimen consisting of ceftriaxone and azithromycin:
- 500 mg intramuscular in a single dose plus azithromycin 2 gms orally as a single dose
- Treatment failure after the single-therapy regimen or other non-preferred treatment regimens:
Ceftriaxone Complicated and mild to moderate community-acquired-intra-abdominal infection in combination with metronidazole:
- 1 to 2 gms intravenous once or twice daily every for 4 - 7 days.
Rocephin use in the treatment of Lyme disease:
- 2 gms intravenous once daily
- for 14 days in neurologic disease,
- 21 to 28 days in carditis, and
- 28 days in arthritis with neurologic manifestations.
Rocephin Use in the treatment of Bacterial Meningitis:
- As empiric therapy in community-acquired bacterial infections or in the following bacterial infections:
- Cutibacterium acnes,
- H. influenzae,
- N. meningitidis,
- S. agalactiae,
- S. pneumoniae with MIC ≥0.12 mcg/mL, and
- susceptible gram-negative bacilli
- 2 gms intravenous twice daily for empiric therapy in combination with other drugs.
Off label use in the treatment of invasive Meningococcal disease, and for prophylaxis of high-risk contacts:
- 250 mg intramuscular in a single dose
Use in treatment of native vertebral Osteomyelitis:
- Off-label use in oxacillin susceptible Staphylococci:
- 2 gms intravenous once a day for 6 weeks
- Off-label use in Beta-hemolytic Streptococci, Cutibacterium acnes or Salmonella spp:
- 2 gms intravenous once a day for 6 weeks.
Note: 6- 8 weeks of therapy is recommended for salmonella infections.
Ceftriaxone use in the treatment of mild to moderately severe Pelvic inflammatory disease:
- 250 mg intramuscular in a single dose plus oral doxycycline with or without oral metronidazole.
Use in the treatment of community-acquired Pneumonia in combination of other antibiotics:
- 1 to 2 gms per day intramuscular or intravenous in 1 - 2 divided doses.
Off label use in the treatment of Proctitis, proctocolitis, and enteritis:
- 250 mg intramuscular in a single dose plus oral doxycycline
Off label use in the treatment of Prophylaxis against sexually transmitted diseases following sexual assault:
- 250 mg intramuscular as a single dose in combination with azithromycin plus metronidazole or tinidazole
Rocephin Use in treatment of Prosthetic joint infection caused by beta-hemolytic streptococci
- 2 gms intravenous once a day for 4 - 6 weeks
Off label use in the treatment of Salmonellosis in HIV-infected patients:
- 1 gm intravenous once a day
- For 7 - 14 days in patients with CD4 count more than 200 cells/mm3 (without concurrent bacteremia)
- More than 14 days (in patients with concurrent bacteremia)
- 2 to 6 weeks regardless of the presence of bacteremia and a CD4 count of fewer than 200 cells/mm3
Off label use in the treatment of Skin and soft tissue necrotizing infections:
- caused by Aeromonas hydrophilia:
- 1 to 2 gm intravenous once a day in combination with doxycycline

Off label use in the treatment and prevention of Spontaneous bacterial peritonitis:

- 1 gm intravenous 0.5 - 2 hours before surgery
- Alternate dosing for colorectal procedures:
- 2 gms 60 minutes before surgery with concomitant metronidazole.
- Cholecystectomy:
- 1 to 2 gms once or twice daily
Off label use in the treatment of Surgical site infections - intestinal, genitourinary tract surgery, surgery of axilla, and perineum:
- 1 gm once a day in combination with metronidazole
Off label use in the treatment of Syphilis in patients allergic to penicillins:
- Primary or secondary syphilis:
- 1 to 2 gms intramuscular or intravenous once daily for 10 to 14 days
- Dose in Neurosyphilis:
- 2 gms once a day for 10 to 14 days.
Rocephin dose in the treatment of Typhoid fever:
- 2 gms twice daily for 10 to 14 days
Use in treatment of complicated Urinary tract infection including pyelonephritis:
- 1 gm intravenous once daily (followed by oral therapy with an appropriate antibiotic for 5 to 10 days.
Off label use in the treatment of Whipple disease:
- 2 gms intravenous once a day for 10 - 14 days, followed by oral therapy (sulfamethoxazole and trimethoprim preferred)

Pregnancy Risk Factor B
- Ceftriaxone(Rocephin).Crosses the placental boundary, but there have been no adverse drug reactions when taken during pregnancy.
- It is used to treat gonococcal and Lyme diseases, as well as other infections in pregnant women.
Ceftriaxone(Rocephin).use during breastfeeding:
- Ceftriaxone enters breastmilk, however, it is considered compatible with breastfeeding. It may modify the infant's bowel flora and may cause GI disturbances in the infant.
- Manufacturer suggests weighing the risks and benefits of breastfeeding against the benefits to the infant.
Ceftriaxone Dose in Renal Disease:
- The manufacturer has not recommended any dose adjustment in patients with liver disease.
- However, the maximum dose should not exceed 2 gms in patients with concomitant renal and hepatic impairment.
- No supplemental doses are required in patients on dialysis.
Ceftriaxone Dose in Liver Disease:
- The manufacturer has not recommended any dose adjustment in patients with liver disease.
- However, the maximum dose should not exceed 2 gms in patients with concomitant renal and hepatic impairment.

Contraindication to Ceftriaxone (Rocephin) include:
- Allergy or sensitivity to ceftriaxone or other cephalosporins or any component in the formulation
- It is best to avoid it in neonates with high levels of bilirubin, especially in premature infants, as it can displace bilirubin.
- It should not be used in conjunction with intravenous calcium-containing products or products for neonates.
- Solutions that contain lidocaine
Warnings and Precautions

Hypersensitivity reactions
- Ceftriaxone can cause severe allergic reactions, especially in penicillin- and cephalosporin-allergic patients. If allergic reactions are detected, therapy should be stopped immediately.

Coagulopathy (Elevated iNR):
- Ceftriaxone therapy may be used to treat coagulopathy, which is a condition characterized by an increase INR. This is especially true for patients who are nutritionally poor or have renal or hepatitis impairment
-
Hemolytic anemia
- Ceftriaxone therapy has been associated with immune-mediated hemolytic anemias
-
Pancreatitis
- Patients who have had a surgery or are sick, as well as patients receiving Total parenteral Therapy, may experience biliary pacreatitis (secondary, stasis of bile).

Superinfection
- Ceftriaxone treatment for longer periods (over 2 months) can lead to superinfections, including fungal or bacterial superinfections. C. difficile-associated diarrhea, (CDAD), and pseudomembranous collitis can occur

Gallbladder pseudolithiasis:
- Ceftriaxone-calcium precipitates may be seen in children. Patients with symptoms that suggest gallbladder disease should stop taking ceftriaxone.

Gastrointestinal Disease:
- Patients who have had colitis or a gastrointestinal condition should be cautious about taking the drug
-
Hepatic and renal impairment
- Patients with concomitant liver and kidney disease should be cautious when taking the drug. Patients with concomitant liver and renal disease should not take more than 2 gms.

Calcium Salts (Intravenous). | CefTRIAXone could produce harmful or hazardous effects. Ceftriaxone is a calcium-binding medication that precipitates as an insoluble substance. Treatment: Neonatal patients who require (or anticipate needing) IV calcium-containing therapies should not be treated with ceftriaxone if they are < 28 days old. For older individuals, flush the lines with a suitable fluid after administration. |
Ringer's Injection, (Lactated). | CefTRIAXone could produce harmful or hazardous effects. In Lactated Ringer's, ceftriaxone binds to calcium to generate an insoluble precipitate. Neonates (28 days or younger) who require (or anticipate requiring) intravenous calcium-containing solutions should be managed. |
Sodium Picosulfate | Antibiotics can reduce the therapeutic effects of Sodium Picosulfate. Patients who are currently using or have just finished using antibiotics should consider using an alternative product to cleanse the bowel before undergoing a colonoscopy. |
Typhoid Vaccine | The therapeutic effects of Typhoid vaccine may be diminished by antibiotics. The only affected strain is the live attenuated Ty21a. Patients being treated with systemic antibiotics should avoid vaccination with live attenuated Typhoid vaccine (Ty21a). This vaccine should not be used until at least three days after the cessation or discontinuation of antibacterial agent treatment. |

- Monitoring parameters while on rocephin therapy:
- Monitor Prothrombin time and INR. Observe for features of hypersensitivity reactions including anaphylaxis.
How to administer Rocephin (Ceftriaxone) injections?
Intramuscular injection of Ceftriaxone (Rocephin):
- Rocephin 250 mg injection must be diluted in a 1:1 water or 1% lidocaine and injected deep into a large muscle mass.
Intravenous injection of Ceftriaxone( Rocephin):
- It should be given as an intravenous infusion over 30 minutes. Intravenous push administration must not a concentration greater than 100 mg/ml given over 2 to 4 minutes.
- Intravenous push administration, especially in high doses (more than 2 gms), may result in tachycardia, restlessness, palpitation, and sweating. It must not be co-administered with calcium-containing solutions.
Mechanism of action of Rocephin (Ceftriaxone):
- Rocephin blocks the formation of proteins, peptidoglycans and other substances by binding to penicillin-binding proteins.
- This eventually leads to cell lysis through the use of autolytic enzymes.
