Tropicamide (Mydriacyl) - Uses, Dose, MOA, Brands, Side effects

Tropicamide (Mydriacyl) causes dilatation of the pupil by blocking the ciliary muscles of the eye to respond to cholinergic stimulation.

Tropicamide (Mydriacyl) Uses:

  • Mydriasis or Cycloplegia:
    • It is indicated for the brief pupillary dilatation (mydriasis and cycloplegia) that is required during diagnostic procedures. For prolonged pupillary dilation, as is required during therapeutic ophthalmic procedures, atropine is used.

Tropicamide (Mydriacyl) Dose in Adults

Note: People with dark and heavily pigmented eyes may require higher doses.

Tropicamide (Mydriacyl) Dose used for Cycloplegia:

  • For treating Cycloplegia (which is when the eye's ability to focus is temporarily paralyzed), tropicamide is given as eye drops.
  • First, 1 to 2 drops of a 1% solution are put into the eye(s).
  • Then, after 5 minutes, if the patient hasn't been examined yet, another drop can be put in.
  • If it's been more than 20 to 30 minutes without an exam, an extra drop can be given to keep the effect going.

Tropicamide (Mydriacyl) Dose for Mydriasis:

  • For treating Mydriasis (which is when the pupils are dilated for an eye exam), tropicamide is given as eye drops.
  • You'd put 1 to 2 drops of a 0.5% solution into the eye(s) about 15 to 20 minutes before the exam.

Tropicamide (Mydriacyl) Dose in Children

Note: People with highly pigmented eyes may require higher doses.

Tropicamide (Mydriacyl) Dose in the treatment of Cycloplegia:

For treating Cycloplegia in infants, children, and adolescents, tropicamide is given as eye drops.

  • You'd start with 1 to 2 drops of a 1% solution, and then wait 5 minutes before repeating the dose.
  • If the patient hasn't been examined within 20 to 30 minutes, you can add another drop to make the effect last longer.

Tropicamide (Mydriacyl) Dose in the treatment of Mydriasis:

For inducing Mydriasis in infants, children, and adolescents, tropicamide is administered as eye drops.

  • Typically, you would put 1 to 2 drops of a 0.5% solution into the eye(s) about 15 to 20 minutes before the eye exam.

Pregnancy Risk Category: C

  • We haven't studied how tropicamide affects animal pregnancies.
  • If eye drops are necessary during pregnancy, it's best to use the smallest effective amount along with blocking the tear ducts to reduce how much gets to the fetus.

Tropicamide use during breastfeeding:

  • We're not sure if tropicamide passes into breast milk.
  • The manufacturer advises caution when giving it to nursing mothers.

Tropicamide Dose in Kidney Disease:

  • The manufacturer's instructions don't mention any changes in dosage for people with kidney problems.

Tropicamide Dose in Liver disease:

  • The manufacturer's instructions don't include any changes in dosage for individuals with liver issues.

Contraindications to Tropicamide (Mydriacyl):

  • If someone is allergic to tropicamide or any part of the medication, they shouldn't use it.
  • While there isn't much evidence of cross-reactivity with belladonna alkaloids, which are similar chemically or in how they work, we can't completely rule out the chance of an allergic reaction.

Warnings and Precautions

CNS effects

  • Tropicamide can lead to drowsiness and blurred vision, which might affect a person's ability to think clearly or react quickly.
  • People using it should be careful when doing tasks that need full mental focus, like driving or operating machinery.

Increased intraocular pressure

  • Tropicamide might cause a temporary rise in pressure inside the eye.

How to administer Tropicamide (Mydriacyl)?

Tropicamide Eye Drops Instructions

  • For Eye Use Only: Only use tropicamide on your eyes.
  • Remove Contact Lenses: Take out contact lenses before using.
  • Reduce Absorption: Press gently on the inner corner of the eye for 2 to 3 minutes after applying to lower how much is absorbed.
  • Avoid Touching: Don't let the dropper touch your eyelids or anything else.
  • Wash Hands: Wash your hands after using the drops.

Mechanism of action of Tropicamide (Mydriacyl):

  • Tropicamide works by stopping the muscles in the iris and ciliary body from responding to a chemical called acetylcholine.
