PCSK9 inhibitors have transformed the management of patients with persistently elevated LDL cholesterol, particularly those with established cardiovascular disease or familial hypercholesterolemia who fail to reach treatment goals despite maximally tolerated statins.
Until recently, PCSK9 inhibitors such as evolocumab and alirocumab were available only as injectable medications. However, in July 2026, the FDA approved enlicitide (Lipfendra), the first oral PCSK9 inhibitor, offering a convenient once-daily tablet. In clinical trials, enlicitide reduced LDL cholesterol by approximately 56–59% when added to statin therapy, with a favorable safety profile.
The availability of an oral PCSK9 inhibitor could improve treatment adherence and expand access to intensive lipid-lowering therapy. While statins remain the cornerstone of cholesterol management, oral PCSK9 inhibition represents an important advancement for high-risk patients who require additional LDL-C reduction.
